Surf camp registration form

This is a concept UI design of surfing camp registration form.

The problem. My client is a surfing camp training business that needs to convert exhibition visitors to potential registrations. Imagine, the user is in a busy and huge international exhibition of leisure sports. He/she scans a special Near-field communication (NFC) code that is on the business card and the form on his/her mobile pop up.

Solution: Having in mind noisy and active exhibition environment I decided to create extremely bold in style, easy to understand representative UI screens. Art direction of the concept is inspired by the brutalism, bauhaus boldness and minimalism.
