Origin configuration

We updated the the visual hierarchy and confirmation/instructional copy that is displayed after activating new versions to add friendliness and clarity. Thanks to @Elias Stein for the spot illustration!

We also changed the "Add a host" form so that instead of a 16-field form, it's only 1 field.
