Resort Feedback Form & Thank You Page

As an intern way back April, 2018, I had the chance to design for one of the awesome group of companies here in Cebu. The Abaca Group are famous for their food and coffee. They also have this exclusive resort which I was able to make the feedback form. It was an amazing experience going through a lot of iterations. I don't think this design retained but I'm still proud of it, what do you guys think? __ __
