DailyUI 001: Hypothetical Form for LinkedIn

The brief was officially to design a modal or form for a signup flow but I decided change things up to imagine a form/modal for a hypothetical feature that allows one to ask stalkers on LinkedIn to state the person.

I can't be the only one who wishes there was some way of knowing why someone is stalking you on Linkedin right?
