Collecting Qualitative Feedback

The product team wanted qualitative feedback on the user's experience of the Tuune health assessment, so I designed this short quiz inspired by the Uber Eats app feedback mechanism.

The optional quiz is designed to be completed in the time it takes for the user's report to load, and once they respond with a thumbs up or thumbs down, they can provide more detail with the chips, or with free text by clicking "tell us more".

In this way, we were able to collect a large quantity of feedback, with minimal disruption to the overall flow.
