Vitals Tracking App - Onboarding Journey Map

This map is a result of data gathered during user research and several co-creation sessions with stakeholders. User base consists of mainly low-income, elderly people, with one or more chronic conditions and the purpose of this (telehealth) service and the app is to help them improve and maintain their health and gain control over their wellbeing. The map was one of many tools developed for this project. It was very useful as a tool for innovation during ideation sessions and was also great for all stakeholders and team members to be on the same page and understand the mechanics of each touchpoint along the journey.
