Spark Health Survey

Spark provides Medicare patients with an advisor to help them understand their options and get the most out of their plan. I designed a personalized health survey used to collect & confirm patient details.

To create an accessible experience the survey features a summary view with the sections needed to complete, individual step views where a question or a set of details is presented, and interstitials used to encourage progress & indicate success. This pattern allows a patient to tap through step by step, or bounce around at their own pace.
