Spark Enrollment Postcard

Spark provides Medicare patients with an advisor to help them understand their options and get the most out of their plan. I designed a personalized health survey used to collect & confirm patient details.

As the first touchpoint of the enrollment, this paper postcard is mailed to patients from their advisor to kickstart the process. Patients can optionally use the unique access code found on the postcard to authenticate, allowing much of the survey to be pre-populated with their existing medical info to give the patient a head start.
