Sport Fansclub Register Page

I just joined for #100days UI challenge from DailyUI.com and this is my first design.

The challenge is

Create a sign up page, modal, form, or app screen related to signing up for something. It could be for a volunteer event, contest registration, a giveaway, or anything you can image.

And in this opportunity, I create a signup/register page for a sport fanclub website. This website is about a fanclub for a specific sports club which has several features such as forums, news, and entertainment for the fanclub related to the club itself.

If you have any idea for me to improve, please comment on this shot. I'm hearing all valuable suggestion. Thanks!
