Multi-Selection and Filtering UI in Custom Reports Creation Form

It's a challenge to handle lots of items from different unlimited data entities/sources when you try to provide users the ability of multiple selection. This becomes even more complex when you have many filtering options to consider.

I used a two-row filtering UI to be able to accommodate as many as chosen by the user and give some flexibility for future added filter types.
